Program

Caring for an elderly family member in the home can be both rewarding
and exhausting at the same time. Despite the caregiver's devotion
to their loved one, sometimes a break becomes a necessity. This
break can be a planned vacation or simply some time alone to rejuvenate.
Burnt Tavern Rehabilitation & Health Center is able to care
for an individual on a short term basis, known as respite care.
Respite care is also ideal for a post same day surgical patient
who is apprehensive about returning to home immediately after
their procedure. Respite stays can be as short as five days or
as long as needed.
Regardless of the needs of an individual, Burnt Tavern is able
to care for them. We have registered nurses on the premises 24
hours a day/7 days a week. Since most respite stays are planned
in advance, our interdisciplinary team is able to customize the
care prior to the actual admission. The needs of the resident
are identified and adjusted throughout their stay, regardless
of the duration.

Hospice
Care
Burnt
Tavern Rehabilitation and Health Care Center offers dignified
and comfortable hospice services for the resident approaching
the end-stage of his or her illness, for patients whose pain or
symptoms cannot be managed at home or who do not wish to remain
at home.
We
offer a home-like atmosphere, and work together with the family
to ease the physical, emotional, and spiritual pain that accompanies
the dying process. We advocate and use a comprehensive range of
services to provide compassionate care for terminally ill residents
including: Effective pain management; palliative/supportive
care to residents and their families; maintain continuity of care
and minimize change by allowing residents to remain in long term
rooms; full range of social, spiritual, and pastoral services;
admission for short-term respite care. |

| Dementia
Unit

|
Our
Dementia Unit services include the Cozy Corner Program and Dementia
Dining Program designed to celebrate the spirit of our residents.
We strive to offer a comfortable and compassionate environment that
emphasizes positive life experiences and active involvement in a
daily routine. The staff who care for our dementia residents are
focused on meeting their special needs. We encourage families to
remain integrally involved in the lives of their loved ones and
offer ongoing support through individual care plan conferences and
support group meanings. |

Medical
Unit

The
Medical Unit provides 24-hour licensed skilled nursing by dedicated
professionals who combine medical expertise with a caring spirit.
Our nursing department offers wound care management and palliative
care. A registered dietician provides nutritional support and
plans meal service.
All
care and services are provided by a highly skilled interdisciplinary
team of professional caregivers under the direction of a physician. |

Recreational
Services

At
Burnt Tavern, we offer a large variety of activities for our residents.
We provide social, emotional, physical, mental, and spiritual
support for all our residents. We encourage participation at all
programs. We cater to meeting the needs of everyone by giving
support and stimulation. We have varied activities including music
programs that features live entertainers. The residents enjoy
singing and playing instruments along with the music. We also
offer group sing-a-longs where staff members join in the fun.
We, of course, play bingo offering prizes. We also provide range
of motion exercise class to maintain physical strength.
We
include all our residents when it comes to activity participation.
Our goal is to provide the best kind of stimulation to all residents.
Our Activities Program is one of they areas we are most proud
of, as it helps to set us apart from a traditional "nursing
home". Our Activities staff is dynamic, energetic, enthusiastic,
and enjoys giving one on one personal, compassionate attention
to each resident. |

Healthcare Center offers a variety of unique, community outings
designed to promote social and emotional well being. Examples include:
Games, Exercise Programs, Social Get-Togethers, Learning Activities,
Creative Pursuits, Spiritual Opportunities, Special Events (weekly
entertainment, field trips, seasonal activities), Pet Visits. |
